- Fill in your personal information, district, and tehsil.
- Turn on location settings so your address is verified.
- Enter your digital wallet account (EasyPaisa, JazzCash, U-Paisa, etc.).
- Explain your situation:
- If your survey was completed but you did not get the card, mention that.
- If your survey was not done, explain that you are eligible and need assistance.
- Upload pictures of your CNIC (front and back) and submit.
